Coach Board Founding Coach Survey Drawing

1. No purchase necessary

NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.

2. Sponsor

AllPlayCoach, Inc., [[ADDRESS]] (“Sponsor”).

3. Eligibility

Open to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older as of the date of entry. Void in Puerto Rico, all other U.S. territories and possessions, and wherever prohibited. Employees, officers, directors and immediate family and household members of Sponsor are not eligible.

4. Entry Period

Begins [[START_DATE]] at 12:00:00 a.m. Pacific Time and ends [[END_DATE]] at 11:59:59 p.m. Pacific Time (the “Entry Period”). Sponsor’s clock is the official timekeeping device.

5. How to enter

Complete the Coach Board survey at allplaycoach.com during the Entry Period, including a valid email address. Limit one (1) entry per person and per email address for the entire Entry Period. Entries that are incomplete, illegible, or submitted by automated means are void.

Alternate Method of Entry: mail a 3x5 card with your full name, address, email address and date of birth to [[ADDRESS]]. Mail-in entries must be postmarked by [[AMOE_POSTMARK_DATE]] and received by [[AMOE_RECEIPT_DATE]]. Limit one mail-in entry per person. Mail-in entries receive the same chance of winning as online entries.

6. Prize

[[NUMBER_OF_WINNERS]] winners. Each winner receives one (1) Coach Board device. Approximate Retail Value (“ARV”) of each prize: [[ARV]]. Total ARV of all prizes: [[TOTAL_ARV]]. Prize is awarded “as is” with no warranty beyond the manufacturer’s standard warranty, if any.

Prizes will be shipped following the first production run and no earlier than [[SHIP_NO_EARLIER_THAN]]. Sponsor makes no guarantee as to the specifications, size, or appearance of the device, which are not final. If the device is not produced, Sponsor will substitute a prize of equal or greater value.

7. Winner selection

Winners will be selected in a random drawing from among all eligible entries received on or about [[DRAW_DATE]]. Odds of winning depend on the total number of eligible entries received. Drawing conducted by Sponsor, whose decisions are final and binding in all matters.

8. Winner notification

Winners will be notified by email at the address provided at entry within [[NOTIFY_DAYS]] days of the drawing. A winner must respond within seven (7) days of notification. Failure to respond, a returned or undeliverable notification, or non-compliance with these Official Rules results in forfeiture and an alternate winner may be selected. Winners may be required to sign an Affidavit of Eligibility and Liability/Publicity Release before the prize is awarded.

9. Taxes

All federal, state and local taxes on a prize are the sole responsibility of the winner. Winners of prizes valued at $600 or more in a calendar year will be issued an IRS Form 1099.

10. Privacy

Information collected at entry is used by Sponsor to administer this drawing and, where the entrant consents, to send product updates. Sponsor does not sell entrant data. Survey responses are used in aggregate to inform product decisions. See Sponsor’s Privacy Policy (opens in a new tab).

11. General conditions

Sponsor reserves the right to cancel, suspend or modify this drawing if fraud, technical failure or any other factor beyond Sponsor’s reasonable control impairs its integrity or proper functioning. By entering, entrants agree to be bound by these Official Rules.

12. Release

By entering, each entrant releases Sponsor and its officers, directors, employees and agents from any liability for any injury, loss or damage arising from participation in this drawing or acceptance, possession or use of a prize.

13. Disputes

These Official Rules are governed by the laws of [[GOVERNING_LAW]] without regard to conflict-of-law principles.

14. Winners list

For a list of winners, send a self-addressed stamped envelope to [[ADDRESS]] by [[WINNERS_LIST_DATE]].
